Each of these forms are State forms that the District must comply with according to State Law. Use them to bring the District into compliance!

OHSA Complaint Form

Safety hazards at your school site that administration has not taken care of, i.e., lighting, unmarked outward opening doors, elevators, exit signs, etc. can be reported to CAL-OSHA. The law states that the complaint will be investigated within 14 days. An example is the exit signs that were broken off by the experimental night high school at LHS in 2000-2001 have never been replaced. The federal and state law is very specific as to the maintenance of exit signs being present and lighted.

Williams Legislation Brochure

The Williams ruling was not only for parents, students, and community but also for teachers and staff. If your room is continually too hot or too cold for teaching then that is a Williams complaint. This brochure gives you all the information to file a complaint.

Request for Information of Public Record

Use this form to request Public Record Information from the District and/or Board guaranteed by the passage of Proposition 59 in 2004. An example would be to request the names and held credential of each of a school site's teachers. This could be used to identify substitutes and vacancies on individual campuses. Under the Williams Settlement every classroom is to be staffed by a credentialed teacher NOT a substitute. This is also helpful in determining the staff at your campus who are not classified correctly, i.e., HR classifying as Probationary 0, a teacher who holds a preliminary, provisional, or clear credential. These are all violations of your rights, your fellow teachers' rights, and the law.
